Chicago Public Schools is excited to welcome its students, their families, and members of the surrounding community to receive school-required and recommended health services, including, but not limited to immunizations. Insurance information is needed for some services (private or public-Medicaid/Medicare).

Minors must have a signed consent form to participate or a guardian present.
Consent forms are attached below or can be picked up in the Nurses Office or at the Main Entrance Door #1.
